"You blew it, Melvin." This is in the episode where Naru and Umino enter that friendship/love contest. The problem isn't the line itself, but that it isn't connected to anything. Like she just shatters his self-confidence randomly.
The Music of the Night and Tuxedo Mirage are what keep me from going completely INSANE! (right...)

The whole scene in R with the "sticky bubblegum stuck in Serena & Dariens teeth" was stupid but that's too many lines to mention. -_-

Hmm, pretty much all the "In the name of the moon I will punish you"-variations are "O_O" but I'll go with the one in episode 68 (w/e the english nr is, I don't know):

"Oh your pretty lipstick cannot hide...all the evil deep inside!" (to Koan/Catzy)

Seriously that comment was hilarious.

Mostly I can take the good with the bad, but some of Serena's screams can get really grating (S season is really bad...I can't remember though if they got worse or better in SuperS).

Imperium Silver Crystal<-----was this even necessary? and then they kept changing to Silver Moon Crystal and other things that I can't remember but if you watch the end of episode 19 of Sailor Moon Abridged they have several clips of this.

In the super s episode with Makoto's friend who wrote the book about Pegasus. Anytime Tigerseye uses the phrase "Babe" but in particular the line "Major hot guy like me" how embarrassing.

And in episode 138 with Miharu where Tigerseye says "I'm Tigerseye the special guy." That line makes me cringe, it's just so tacky.
