UNFORGIVABLE mistakes in the dub

Which do you think was the worst? Here are some that make me shudder:
In "Match Point for Sailor Moon", when MOLLY was at SERENA's house and MOLLY said "Thanks for coming over".
At the start of "Molly's Folly" when Molly was having flashbacks of Nephlyte and calling him 'Nephlyte', not 'Maxfield'.
The Sailor Says segment of "Day of Destiny" when all their voices were mixed up.
In "Trouble Comes Thundering Down", when Luna P (Sailor Pluto) is talking in Luna's voice.
In "Follow the Leader" when Raye argued that she was a Sailor before Amy.
Anyone think of any other really bad ones?
