The way Minako's crush figured out who she was was because of her volleyball move, that diving rolling thing. He recognized that move from Minako. You even hear him say... I think the quote goes: " I know that move!" And you get a vision of Minako doing the move, then as Minako goes through it she transforms into Sailor Venus. Perhaps because Fiore was so jealous of Usagi that his burning hatred towards her left such a clear image of her in his mind that he could instantly recongize her even when she was transformed.
